Orta hat diasteması ve arayüz çürüklerinin direkt kompozit rezin ile estetik restorasyonu

Abstract

Amaç Estetik beklenti ile başvuran 19 yaşındaki hastada, çürükler uzaklaştırıldıktan sonra silikon indeks rehberliğinde üst anterior dişlerde (#11, #12, #21, #22) direkt kompozit restorasyonlarla diastema kapatma, simetri ve renk uyumunun sağlanması amaçlandı. Olgu Tanımlanması Muayenede #11, #21, #22’de aktif dentin çürüğü ve #11–#21 arasında diastema; endodontik tedavili #12’de sekonder çürük saptandı. İlk seansta polisaj yapıldı ve C tipi silikon (Oxasil, Kulzer, Almanya) ile ölçü alınarak alçı model üzerinde mum modelajı sonrası silikon anahtar hazırlandı. Lastik örtü (Nictone Heavy Blue, Velopex, İngiltere) altında enfekte dentin uzaklaştırıldı. Selektif asitleme tekniğiyle mineye %37 ortofosforik asit 30 sn uygulandı. Ardından adeziv ajan (3M Scotchbond, Almanya) 20 sn ovalanarak uygulandı, hafif hava ile 5 sn kurutuldu ve LED ışık cihazı (Led-e Plus,Woodpecker,Çin) ile 20 sn polimerize edildi. Silikon anahtar kullanılarak palatal mine duvarı A2 renk kompozit ile (G-ænial A’CHORD, GC, Tokyo, Japonya) oluşturuldu. Dentin kompoziti AO2 (G-ænial, GC, Tokyo, Japonya) tabakalı teknikle yerleştirildi. Fasiyal yüzeyin son tabakası A2 renk kompozit ile tamamlandı. Her kompozit tabakası 20 sn polimerize edildi. Anatomik düzenleme düşük devir kırmızı bantlı elmas frezler ve cila diskleri (Sof-Lex, 3M, ABD) ile, son cila iki renkli spiral disklerle (Twist Diacomp Plus, EVE, Almanya) yapıldı. Hastaya ağız hijyen eğitimi verildi. Bulgular Restorasyonlar Modifiye Birleşik Devletler Halk Sağlığı Servisi (USPHS) Kriterlerine göre değerlendirildi. Altı aylık takipte kırık, yüzey defekti veya renk değişimi gözlenmedi; fonksiyon ve estetik uyum tatmin ediciydi. Sonuç Silikon indeks rehberli direkt kompozit restorasyonlar, anterior bölgede diastema kapatma ve simetri-renk uyumunun sağlanması gereken olgularda minimal invaziv, hızlı ve hasta memnuniyeti yüksek bir tedavi seçeneğidir.

Aim In a 19-year-old patient who presented with esthetic expectations, after removal of caries, it was aimed to close the diastema and achieve symmetry and color harmony in the maxillary anterior teeth (#11,#12,#21,#22) using silicone index–guided direct composite restorations. Case Description On examination, active dentin caries were detected in teeth #11, #21, and #22, a diastema between #11–#21, and secondary caries in endodontically treated #12. At the first appointment, prophylaxis was performed and an impression was taken with C-type silicone (Oxasil,Kulzer,Germany). After wax-up on the plaster model, a silicone index was prepared. Under rubber dam isolation (Nictone Heavy Blue,Velopex,UK),infected dentin was removed. Selective etching was performed by applying 37% orthophosphoric acid to the enamel for 30 seconds. Then, an adhesive agent (3M Scotchbond,Germany) was applied with rubbing for 20 seconds, gently air-dried for 5 seconds, and polymerized for 20 seconds with an LED light-curing unit (Led-e Plus, Woodpecker, China). Using the silicone index, the palatal enamel wall was formed with A2 shade composite (G-ænial A’CHORD,GC, Tokyo, Japan). Dentin composite AO2 (G-ænial,GC,Tokyo,Japan) was placed using the layering technique. The final facial layer was completed with A2 shade composite. Each composite layer was polymerized for 20 seconds. Anatomical contouring was performed with low-speed red-stripe diamond burs and polishing discs (Sof-Lex,3M, USA), and the final polishing was completed with two-step spiral discs (Twist Diacomp Plus,EVE,Germany). The patient was instructed on oral hygiene. Results The restorations were evaluated according to the Modified United States Public Health Service (USPHS) Criteria. At the six-month follow-up, no fracture, surface defect, or color change was observed; functional and esthetic harmony was satisfactory. Conclusion Silicone index–guided direct composite restorations are a minimally invasive, fast, and highly satisfactory treatment option for cases requiring diastema closure and achieving symmetry and color harmony in the anterior region.
